The concept is simple, GatewayMD will send you a blood pressure cuff that automatically sends your readings to our clinical staff. You will be assigned to a dedicated care nurse and Nurse Practitioner that will communicate with you throughout the month while tracking your BP readings daily. If your care team notices an elevated reading or a negative trend, they will reach out to you. From there we will give guidance, and even contact your doctor if need be.
